Upgrade Medical, LLC and its associated providers have opted out of all commercial insurance plans as well as both Medicare and Medicaid programs (including OHP or other plans providing Minimum Essential Coverage (MEC) as defined under the Affordable Care Act).
This includes workers' compensation, motor vehicle, homeowner, or other such policies.
Under the No Surprises Act (H.R. 133 - which went into effect on January 1, 2022), health care providers need to give clients or patients who do not have insurance or who are not using insurance an estimate of the bill for medical items and services.

You are also welcome to request a Good Faith Estimate at your visit. This Good Faith Estimate shows the costs of items and services that are reasonably expected for your health care needs for an item or service. The estimate is based on information known at the time the estimate was created. You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes (under the law/when applicable) related costs like medical tests, prescription drugs, equipment, and hospital fees. The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment.
To ensure accuracy it is best to do this in person with your provider so a GFE can be created based on your specific goals and or needs. For most consultations and standard procedures we can also generate and provide you a "Superbill" for services rendered following payment to submit to your insurance, FSA, or HSA, but coverage or reimbursement is NOT guaranteed.
We do our best to ensure these prices remain accurate but there are things outside of our control such as supply or shipping cost changes. It is also important to note that if you utilize an outside lab, we cannot guarantee pricing. We utilize Rupa as our lab service and payment is made directly to them. Costs are listed in the store. This is also true for our supplement supplier Fullscripts.
