Step 1: Visit: https://upgrademedical.md-hq.com/registration (save this site)

Step 2: Enter your information, including meds etc. Complete all forms and consents

Step 3: Create Patient Portal Account (for already registered Patients)

Step 4: DOWNLOAD APP ( Use the same password from Cerbo- don't re register) For now here you will add your cc in payment gateway (if doesn’t load the 1st time just go out and back in).
* Fee will be waived if using US Debit or HSA *
